  • Please complete entire application online including application form, menu and wine list document uploads, and application fee.
  • All three steps must be completed to be eligible.
  • Make sure to upload a copy of your dinner menu and wine list in either PDF or Word Document format to complete step two.
  • Application fee: $25, paid online during third step of registration.
  • No faxes or emails accepted-full application must be submitted online. Please submit by deadline of July 19, 2010.
